The defendant requires that the following question or issue, viz. [here state the question or issue required to be determined] should be determined not only as between the plaintiff and the defendant but also as between either or both of them and yourself.

And take notice that [as in No. 20 down to the words "intention to contest the proceedings"] you will be bound by any judgment or decision in the action so far as it is relevant to the said question or issue, and the judgment may be enforced against you in accordance with Order 16 of the Rules of the Supreme Court Chapter 4.
