We, the parties to a *customary/validated marriage particulars of which are given below, hereby give you notice in accordance with section 16(1) of the Marriage Reform Ordinance of our intention to dissolve our marriage in accordance with Part V of the Ordinance.

選啟者:余等乃締結*舊式婚姻/有效之新式婚姻之雙方,其有關情形詳載下文,茲特依照修訂婚姻制度條例第十六款第一段向台端通知:余等擬依照該條例第五部之規定將余等之婚姻解除
