We, the parties to a *customary/validated marriage particulars of which are given below, hereby give you notice in accordance with section 16(1) of the Marriage Reform Ordinance of our intention to dissolve our marriage in accordance with Part V of the Ordinance.

2. Our marriage, which took place on the day of 19...., has been registered in accordance with Part IV of the Ordinance and was subsisting on the appointed day (7.10.1971). A copy of the certificate of marriage (issued under section 9 of the Ordinance) is attached.
