(1)
I understand that the effect of an adoption order is to deprive a parent or guardian of all rights in respect of the maintenance and upbringing of the infant.
(一) 余知領養令有解除父母或監護人對該幼童撫養及教養之一切權利之效力。
(2) I understand that, when the application for an adoption order in respect of the said A.B. is heard by the judge, this document may be used as evidence of my consent to the making of the order unless I have notified the court that I no longer consent ‹*).
(二) 余知當地方法院法官聽取關於該A.B.之領養令之申請時,本文可用以證明余係同意頒發是項領養令者,除非余事前通知法院不再同意(註六)。

I hereby consent to the making of an adoption order in pursuance of the said application [on condition that the religious persuasion in which the infant is brought up is ....].
(三)余今同意[在該幼童本身之宗教信仰條件下](註七)根據該項申請頒發領養令!
