BOXES 8 AND 9
For state servants, insert the total length of state service followed by the length of service in the present grade. Where the recommendation is for services in one particular appointment, insert the length of time in that appointment. For non-state servants, insert the total length of service in the position, or positions, to which the recommendation refers.
BOX 10
If the candidate is a state servant who is expected to retire within the next two years, insert the date (month and year) of retirement. If appropriate, indicate whether retirement is from established service or any subsequent disestablished service. If the candidate is not a state servant and has retired or is expected to retire within the next two years from the position relative to the service for which he/she is being recommended, insert the date (year and if possible month) of retirement.
BOX 11
If the candidate is a non-state servant, insert the name and occupation, if any, of his/her spouse.
BOX 12
Insert, if known, the occasions (if any) on which the candidate has been unsuccessfully recommended for the proposed award in the past e.g. "BD '85, NY '86".
BOX 13
Insert any British honours the candidate already holds and the year of their award, if known. For candidates for honorary awards, insert also any honours (including grade) the candidate has received from other countries, if known.
BOX 14
If the candidate is being recommended for consideration on the Prime Minister's List, insert his/her private UK address.
BOX 15
If more than one candidate is being recommended for the same honour, insert the order of preference i.e. "(1)", "(2)", "(3)" etc.
BOX 16
If the candidate is a state servant, insert the national salary scale for his/her grade i.e. salary less London Weighting. For locally engaged staff simply indicate the grade e.g. LE I, LE II etc.
BOX 17
Insert the name (not the signature) and the appointment of the FCO/DS`recommender e.g. "John Smith, HMA Utopia", or, for recommendations from outside bodies, the title of the recommender and the name of the recommending organisation e.g. "Director, British Council".
BOX 18
Insert the date when the recommender made or approved the recommendation.
BOX 19
Insert a comprehensive but succinct description of the reasons why the candidate is being recommended for this particular honour, concentrating on what meritorious services to the State or to specific British interests were performed, for how long they have been, or were, performed and what specifically makes them worthy of recognition by means of an honour. A full CV is normally unnecessary.
