HOW TO COMPLETE THE FCO HONOURS RECOMMENDATION FORM

INTRODUCTION

1.

This form is for use by all those who submit recommendations through the FCO for British honours, both substantive and honorary and, for substantive awards, whether for consideration on the Diplomatic Service and Overseas List or the Prime Minister's List. It should be completed in typescript.

2.

All relevant boxes should be completed. But if the answers to some questions are unknown and cannot be ascertained without great difficulty, please insert "N/K".

3. Where a bald answer to a question, or questions, would seem likely to give an inaccurate or incomplete impression, please use the narrative at box 19 to add any necessary amplification or explanation.

4. The form carries the privacy marking "Honours in Confidence". If the subject matter of a completed form justifies a security grading this should be added.

BOX 1

Indicate whether the candidate is a state or non-state servant. A state servant (or "official") is anyone in the employment of (i.e. paid by) the Crown. For example, any UK-based or locally-engaged (whether British or foreign) member of the DS, FCO, British Council, ODA (including OSAS etc experts) HCS or HM Forces. In the Dependent Territories, any member of the Administration or Government employee.

A non-state servant (or "non-official") is anyone not in the above categories as well as British members of international organisations e.g. the UN or EC. If in doubt, leave the box blank.

BOX 2

For substantive awards only (honorary awards can be submitted at any time) indicate whether the candidate is to be considered for the New Year Honours List (1 January) or the Birthday Honours List (early June) and the year.

BOX 3

Insert the candidate's title, if any, or civil status (Mr. Mrs. or Miss) followed by forenames, in full, and sumame.

BOX 4

Insert the honour for which the candidate is being recommended. If in doubt about the appropriate award or level, an alternative award or level may be indicated.

BOX 5

Insert the candidate's precise citizenship or nationality status. The description "British subject" no longer has the wide meaning it was given before the introduction of the British Nationality Act 1981 and should be used to describe only those persons who are British subjects under Section 30 and 31 of the Act. Other British nationals and foreign or Commonwealth citizens should be described by their correct status, i.e. British citizen, British Dependent Territory citizen, British Overseas citizen, British Protected person, British national (overseas) or citizen of Canada, citizen of France etc. Any second nationality or citizenship should always be stated. (Candidates who are not The Queen's subjects, including foreign LE staff, are eligible for honorary awards only.)

BOX 6

If the candidate is a state servant, insert his present rank or grade and his appointment e.g. "DS.5, Head of Chancery, Utopia" or "HCS 3, Minister (Defence Sales) Utopia". If the candidate is not a state servant, describe the position, or positions, he holds relative to the recommendation e.g. "Managing Director, BP Ruritania and President of the UK/Ruritanian Chamber of Commerce".

BOX 7

Insert the candidate's age in years at the time of the recommendation.
