APPLICATION FOR RECEPTION AS A

VOLUNTARY PATIENT.

Asylums Ordinance, 1936, s. 8A,

hereby apply to be admitted to the mental hospital as a voluntary patient. =

I undertake, as from the date of my admission-

(a) To pay all fees charged by the mental hospital for maintenanc and treatment;

(b). To conform to all the regulationáj

of the mental hospital;

(c) To give the M.O.ifc 72 hours! notice of my intention to leave the mental hospital;

(d) Forthwith to leave the mental hospital when so required by the M.0.1/c;

(e) To make no claim against the mental hospital in respect of lost or damage to any property brought into it (including private clothing).

"Penalties.

38. Any person who acts in con- travention of regulations 3, 5, 110) 12 to 14, 19(1) and (2), 21(1), 22, 23, 24(1) and (2), 23(3), 28, 30, 31, 32, and 37 shal, on summary conviction, be liable to a fine of five hundred dollars, and, in the case of a continuing offence, to a Further fine of one hundred dollars for every day subsequent to the day on which he is convicted of the offenco during which the contravention or default continues.*

The regulations appearing om pages 876 La 880 of Volume II of the Regulations of Hong Kong (1937 Edition) under the heading "Dealers in Marine Stores" are amended by the addition after regulation 7 of the following-

PENALTY.

8. Any person who acts in contraven- tion of regulations 2, 3, 4, 5 and 6 shall be liable to a fine of one thousand dollars or to imprisonment for three months."'

The Dogs and Cats Regulations published in the Gasette of the 13th January, 1950, as Govern- ment Notification G.N.A. In are amended by the addition after regulation as of the following-
