ANKYLOSTOMIASIS CAMPAIGN.

International Health Commission.-Government of Antigua.

Ar the instance of the Director-General of the International Health Commis- sion and the Acting Director for the West Indies I proceeded to Antigua.

I arrived in Antigua 1st March. Official confirmation of my appointment as Director of the Antigua Ankylostomiasis Campaign was signed by His Excellency the Acting Governor on 8th March.

The first week was given over to conferences with Government officials and reviewing the work done in the York Valley District, the report of which you already have.

The second week was spent in selecting a new area for work and moving the office and laboratory equipment. The area selected was one comprising the villages of Bendals, Green Castle, Hamiltons, Brecknocks, Sawcaults, and John Hughes. By taking the These villages are contiguous to the ones in the York Valley. villages in this order we take advantage of the knowledge of the work, which is spread by discussion of it, among the inhabitants.

Illustrated talks on the cause and spread of the hookworm disease were given in the Bendals schoolroom and the Sawcaults schoolroom, the only two public buildings in the area.

His Excellency the Acting Governor addressed the people at the Bendals school. His Excellency's presence at the meeting, and the interest that the Government is taking in the work, will aid materially in the success of the campaign.

At both lectures the doors and windows, as well as the aisle, were filled with people who could neither find sitting nor standing room. Working among people of such evanescent ambition, it is highly desirable to catch them at the crest of their enthusiasm to get the best results.

The actual field work of the campaign was started on 17th March, when census-taking began in all the villages.

The sanitary survey shows three houses in John Hughes, and one house in each of the villages of Bendals, Hamiltons, and Sawcaults that have a privy. In the villages of Brecknocks and Green Castle there are no privies of any sort.

The privies found were all of the "pit latrine" type. They were all well housed over properly constructed pits, with the sides well banked or other means used to The privies located on a steep prevent surface water from flooding the pit. declivity were surrounded on the up hill side by a ditch, which would carry water flowing down the hill away from the privy, so that the pit would not become flooded.

Two cases of very heavy infection with Stronglus subtilis were found. people were advised to consult their family physician, who was also advised of our microscopical findings. Two cases of yaws and one suspected case of leprosy were also reported to the proper local authorities.

THE work of the ankylostomiasis campaign during the past month has been an extension of the work started last month.

Census-taking has been completed in all the villages. The only additions likely are people who move into the area before the campaign here is closed. The total number of people in the "Belvidere District is 880.

All of these people have been examined who are likely to be examined. Only one refusal of examination has been met; persons under treatment for other diseases are not examined, except with the consent of the attending physician. A spirit of procrastination is everywhere present, but this will not seriously delay the work. Of the 820 persons examined 238 were found infected with hookworm. This makes a rate of infection of twenty-nine per cent. for this group of villages.

Of the 238 persons who were found infected, 222 have started treatment. Of those taking treatment sixty-two have been cured. Fifty-three were cured with two treatments, five with three treatments, and four with four 'treatments.

Thirty-seven persons have been found infected with the Schistosome mansoni. This infection is very rare in any part of the world. The small number of cases reported in the medical literature have mostly come from the West Indies. will be made the subject of a special report when the extent of the infection and the clinical aspects of the disease have been thoroughly studied.

There are five privies in the area, which embraces 880 people. One lecture has been given at the All Saints' school and one at Sawcaults school. The The lectures were given in the afternoon for the benefit of the school children. large hookworm chart was used to illustrate the talks. The attendance at the two lectures was 250.
