(f) 如按摩院將以合夥方式經營,則須交合夥契約副本及有關的商業登記證副本;及

(g) 如按摩院將由一法團擁有,則須呈交由公司註冊官簽發的公司註冊證明書副本。

本人謹聲明,本人在本表格上所填報的資料及詳情,盡本人所知所信,都是正確無訛的。本人已閱讀按摩院條例(香港法例第266章)以及在該條例下制訂的按摩院規例。本人明白該條例第13(3)條規定,任何人在有關申領牌照或申請牌照續期一事上作出虛假或誤導的聲明,或提供失實或誤導的資料,即屬違法,可被判罰款$50,000及監禁6個月。
