A

request made under this Article shall be

accompanied by :

(a) details of any relevant order made for the

purpose of ensuring that property is available

for confiscation;

(b)

statement describing the grounds for believing

that the person subject to the investigation or

proceedings is connected with drug trafficking

or its proceeds;

(c)

a description of the property to be seized and

frozen including so far as possible the location

of the property;

(d) a statement specifying the connection between

the person who is the subject of the

investigation or proceedings and the property to

which they relate; and

(e)

a statement describing

the proceedings which

have been or are about to be instituted, and

where proceedings have not been instituted, when

and

in

what form it is intended that they will
