A request for restraint of proceeds shall be

accompanied by :

(a) a statement describing the grounds for

believing that the person subject to the

investigation

or proceedings has benefited

from drug trafficking;

(b) a description

measures

of the property to which

of restraint are to be applied

including as far as possible the location of

the property;

(c) a statement
