the purpose of the request, including a description of the evidence, information, or other assistance sought;
the nature of the investigation, prosecution or proceedings in relation to which the request is made.
A request for freezing or restraint shall also include:
a statement describing the grounds for believing that the person subject to the investigation or proceedings has benefitted from drug trafficking;
