ARTICLE V

FORM AND CONTENTS OF REQUESTS

1.

Requests shall be submitted in writing except in

urgent cases, In urgent cases, a request may be made orally, but shall be confirmed in writing within ten days thereafter.

2.

A request

made pursuant to this

Agreement

shall

state that it is so made and shall include the following:

(a) the

the

authority conducting

the

Investigation,

prosecution or

proce eaings to

which the request relates;

(b)

the

(C)

ot purpose

the

request, including

a

description of the assistance sought; and

a description of the nature of the

investigation,

prosecution or proceedings in

relation to which the request is made.

3.

A request for treezing, restraint or seizure shall

also include :

(a)

a

statement

describing

the

grounds

for

believing that the

subject person

to the

investigation or proceedings has engaged in or

benefited from drug trafficking;
