In compliance with specific instructions from NNSA and the Executive Committee, the JVC organised its PRC

JVC organised its PRC and expatriate experts to examine the technical solution proposed by the EdF designer and the related design information and analyses from the designer. It was established that the technical solution of EdF designere met the design criteria and was equivalent in structural strength, margina and quality to the original design.

A summary of the alternative technical solutions is given in Appendix 3. It is confirmed that the EdF proposal did not represent the cheapest possible solution. There were alternatives which would have satisfied the design criteria at lower cost but which were considered technically less desirable than that proposed by Edr.

The technical solution was issued to NNSA for review, NNSA confirmed verbally on 8.10.1987, and then in writing on 16.10.1987 that they had no objection to this technical solution.

5. Conditions 4 and 5: Improvement of Quality Management

This non-conformance was due to wrong interpretation of drawing by the contractor HCCM. However, it was not discovered by checks made before pouring of concrete. Subsequent investigation showed that quality checks were not strictly enforced by the Contractor, ΟΙ adequately monitored by the JVC's QC surveillance team. The JVC has taken this non-conformance

very serious incident 48 it represented a breakdown in quality management and controls.

The JVC Executive Committee and General Management recognised the seriousness of this non-conformance corporately and publicly. The JVC are very concerned about the failure of the quality management and control functions of the contractor as well as those of the JVC, for not able to prevent or to discover earlier the non-conformance,

On 25.9.1987, Chairman Wang and some other directors of the JVC reviewed the incident with the contractor HCCM at #ite. The First Deputy Chairman, Mr. Stones, and some other JVC directors met the senior management of HCCM's leading partner Campenon Bernard and the Project Service Contractor Edr. In these meetings, the parties concerned were urged to vigorously review the problems which had caused the non-conformance, and to implement immediate measures to strengthen the construction management and quality controls of both IICCM and the JVC to avoid future occurrences.
