Questionnaire
Directions:
1. There are five answers to choose from for each question, but
respondents can tick only one answer.
2.
No names will be recorded, so choose the answer that truly speaks your mind.
3. Consider and answer each question individually.
There is no need to
answer all questions. If you do not understand the meaning of any question, you can refrain from answering.
4. The contents of the questions do not represent the Union's position and the findings of the survey may not necessarily be published. The Union will account to its members as to how the results will be utilized.
5. This questionnaire is an internal document and cannot be shown to
outsiders.
6. Please complete the questionnaire on your own; do not consult others
or cive standard answers.
7. Place in the attached envelope after completing and return to the Union
through the representatives. (You can also send it back to the Union directly)
8. All returns should reach the Union on or before 26.6.84.
9. Any other suggestions can be given on a separate sheet of paper.
10. Non-members are also welcome to complete the questionnaire.
