THE OUTCOME OF ENQUIRIES CONDUCTED BY THE MARINE DEPARTMENT ON WERE ACCIDENTS AT SEA SUGGESTED THAT MOST OF THE CASES OCCUPATIONAL CAUSED BY THE CARELESSNESS OF THE WORKERS INVOLVED OR FAILURE WORKERS AND THEIR FOREMEN TO OBSERVE SAFE WORKING PRACTICES.
