If a plague Inspector has a case of plague, and he

wants three gallons of Jeyes Fluid, he makes out a

requisition form, which he signs himself, and it is

given to my clerk, and the clerk goes to the store,

and takes this out and gives it to him, In the

requisition book, there is a duplicate. The one torn

off is on file, and the other goes into the store.

Are those issued by the Medical Officer in charge

of plague, diptheria, or whatever it is, and if he

wants a lot, he says cases of plagus at so and so and
