If a plague Inspector has a case of plague, and he

requisition form, which he signs himself, and it is
given to my clerk, and the clerk goes to the store,
and takes this out and gives it to him, In the
requisition book, there is a duplicate. The one torn
off is on file, and the other goes into the store.
