The declaration shall contain sufficient information to satisfy the Secretary of State that the declarant-

(a) was granted citizenship of the Republic of Cyprus in pursuance of an application made in accordance with section 4 or 5 of Annex D to the Treaty concerning the Establishment of the Republic of Cyprus before he attained the age of 16 years, and (b) immediately before being granted such citizenship was a citizen

of the United Kingdom and Colonies.
