APPLICATION FOR INCLUSION IN THE FINAL REGISTER OF ELECTORS.
IMPORTANT,
Please read the following notes carefully before completing this Form. Note 1. Name and address in English to be completed in BLOCK LETTERS. Note 2. You may be eligible do claim registration under more than one category.
If so, choose the one under which you prefer to be registered. Note 3. Insert in the appropriate place in this Form, the information required

Note 4. You are required to have a three years' residential qualification, as indicated in the Declaration at paragraph 3 of this Form. Two dates are given in that Declaration, namely "15th February last" and "15th June fast"". If you claim to be registered by vason of being register- ed as an Elector on the last previous Final Register, you should delete 15th June last in paragraph 3(c) of this Form. If you are claim- Ing to be registered for any other reason, then you should delete "15th February last in paragraph 3(c) of this Form. With regard to the three years referred to in paragraph 3c), mere temporary absence from the Colony (whether for business, holiday or any other reason) during that three years does not necessarily mean that you were not ordinarily resident in the Colony for those three years.
Note 5. It is your responsibility to ensure that the particulars you enter on this Form are correct. Your attention is drawn to the DECLARA- TION in paragraph 3 of this Form, which you are required to sign. If you knowingly and wilfully make or sign that Declaration which is untrue in any material particular, you will be liable, on conviction, to a fine of one thousand dollars and to imprisonment for six months,
Note 6. Your attention is also drawn to the fact that if you are registered, you will be registered in that part of the Final Register compiled for the electoral district in which you reside. This means that you will be entitled to vote only at the polling station designated for that electoral district (as shown in the Final Register).
