I undertake that the said
(i) will remain and may be detained in the said centre or in any other
centre to which he may be transferred if required by the superintende of the centre, for a period not exoceding six months from the date of his first admission to the said centre in accordance with this application. (i) will submit himself to such treatment as may from time to time be prescribed by the superintendent of the centre in which he is detained, (iii) will obey all lawful orders given to him by the superiorcade
or by any person authorized by the superintendent;
(iv) if required by the superintendent, will leave the centre in which bei
detained, within forty-eight hours of being so required;
(v) may be visited only by such persons as are permitted by the super
intendent; and
(vi) if given leave of absence by the superintendent, will return to the cas in which he was detained, on the expiry or determination of the period of leave of abszuce.

I acknowledge that he may be detained against his will in the wid ccatre or in any such centre to which he may be transferred, for a period not exceeding six months from the date of his first admission to a centre in accord- ance with this application and that he may be forcibly retaken by the superio- tendent or by any police officer if he absents himself from the centre in which he is detained, without the permission of the superintendent of the centre or having been given leave of absence from a centre by the superintendent, he fail to return at the end or determination of the period of absence.
4. If required I undertake to repay to"
the cost of his maintenance and treatment in such centre as he may be detained, in accordance with such scale as has been shown to me before I completed this application or on such scale as may be prescribed from time to time.
