The Owner or Occupier of No. Take notice that the service to the above mentioned reuerent having on inspectiou been found to be defective, you are requested to take steps to carry out the alterations or repairs set forth in the accompanying memorandum which I hereby certify to be necessary, Should you fail to carry out such altorations and repairs within thirty days after the receipt of this notice, the service will be disconuectal from the Waterworks and will not be re-connected until it is renewed, altered o ropired to my satisfaction. Should you desire it, the necessary work can be done by the Water Authority at your expenses hi such case you or your authorized agent shoukt`call it the Office of the Water Authority to sign the form which will NE supplied to you.
