"The Register" means the Register of Nurses formed and kept by the Board under section 3 of the Ordinance. Registered Nurse means a male or female nurse who is for the time being registered in the Register.

3. The forms set out in the Schedules to these regulations and all instructions contained therein shall be deemed to be part of these regulations, and such forms, or forms sub- stantially to the like effect, shall be used in all cases for which the forms are provided.

PART II.

FORMATION, MAINTENANCE AND PUBLICATION OF THE REGISTER.

4.-(1) The Register shall consist of the following parts:—

(a) A general part containing the names of all female nurses who satisfy the conditions of admis- sion to that part of the Register: (b) A supplementary part containing

the names of male nurses:

(c) A supplementary part containing the names of nurses trained in the nursing and care of persons suffering from mental diseases: (d) A supplementary part containing the names of nurses trained in the nursing of sick children:

(e) A supplementary part containing the names of nurses trained in the nursing of persons suffering from infectious diseases.

(2) The Register shall contain the parti- culars set forth in the First Schedule to these regulations.

(3) Each person admitted to the Register shall be assigned a consecutive number in the part or parts of the Register in which her name is included.

5. 6.--(1) A nurse who desires her name to Fee for

Register. quent to the first year in which it is included in the published Register, shall on or before the thirtieth of September of the previous year forward to the Director of Medical and Sanitary Services a retention fee of two dollars.

(2) A nurse whose name is included in the Register shall notify to the Director of Medical and Sanitary Services every permanent change of address, and for the purposes of these regulations the address last notified shall be deemed to be the registered address of the

nurse.

(3) The Director of Medical and Sanitary Services shall send to the registered address
