I, SARAH MA DAR, of Shanghai, China, widow, a British
subject most solemnly and sincerely declare that: -
1. I was born in the Colony of Hongkong on the 6th day
of January 1873.
2. THAT on the 2nd day of January 1889 I was married
at Fongkong to Omar Abdool Madar, also a British subject,
he having been born at Hongkong on the 22nd day of July
1871.
3. THE said Omar Abdool Madar died at Hongkong on the
14th day of December 1923.

Zobiede Madar who is dead.
Frederick Omar Madar born in 1893.
Margaret Yvette now Margaret Yvette Berthet born
in 1894.
George Madar Madar born at Shanghai 26th April 1896 Kathleen Madar who is dead.
Frances Blanche Madar now Frances Blanche Manley. Charles Madar born at Shanghai 9th April 1901. Patrick Madar born at Shanghai 17th March, 1903. May Madar born at Shanghai 22nd May 1904. Thomas Madar born at Shanghai 21st May 1905. Dorothy Madar born at Shanghai 10th August 1907. Harry Madar born at Shanghai 9th December 1909. Evelyn Madar born at Shanghai 18th February 1913.
THE said Frederick Omar Madar and Margaret Yvette
Berthet having been born in Hongkong their births were
registered there.

THAT the said Omar Abdool Madar neglected to register.
the births of our other children the said George, Charles,
Patrick, May, Thomas, Dorothy, Harry and Evelyn Madar at
His Britannic Majesty's Consulate General at Shanghai
aforesaid.
