present yourself to one of the Medical

Advisers whose names are given on the

enclosed letter, for the purpose of

being medically examined. The letter is

to be left with the Medical Adviser and

he will send his report direct to this

Department. The fee will be paid by the

Government. A Medical History sheet is

also enclosed which should be filled up

and handed to the Medical Adviser at the

time of the examination.
