STATEMENT for the information of the Secretary of State for the Colonies, and for record in Colonial Office, respecting Anna Lazenby who is an applicant for the post of Matron Nurse in the Colony of Hong Kong.

1. Will you be good enough to state how long you have been acquainted with the applicant?

1. From October 1893 to September 1899.

2. From what circumstances does your knowledge of her arise?

2. As Chaplain of the Royal Infirmary Sheffield where she was nurse and Sister for the period above named I had constant opportunity of becoming acquainted with her.

3. While you were acquainted with her was she

5. If she has been employed under you, will you state the nature of her duties, and how she discharged them?

5. All the time as I remember. She was nurse and Sister and I never wish for a better or kinder nurse.

6. Will you state the cause of her leaving-whether by voluntary resignation, dismissal, or otherwise?

6. This Lazenby left the infirmary of her own accord to go to a new place & change of work.

9. The Secretary of State will be glad to receive any information which you may have to give respecting her character or qualifications in addition to what is contained in the answers to the foregoing questions.

9. I always found Miss Lazenby most agreeable to work with both as probationer nurse and during the time she was charge nurse of the Surgical wards. She also nursed men during a long illness and I never knew her to be lazy.
