4. Sisters shall keep a Report Book and, in it, note the condition of their patients and the directions given by the Doctor during their time of duty, for the guidance of the sisters who follow them.
5. Sisters shall daily report to the matron as to the condition of their wards, giving her the earliest information of any serious illness or operations.
