Government Civil Hospital, May 28th, 1899.

I regret to have to report that Miss David, who only joined the Nursing Staff on the 4th March last, has completely broken down in health.

On the 12th instant she complained of insomnia and headache; it then came to my knowledge that she was taking drugs to excess. I took her off duty and as she did not recover on the 17th instant, I transferred her to the Hospital, her mental condition now causing considerable anxiety.

She became subject to delusions and at times was very violent; she could not be left night or day. During the last twenty-four hours she has developed undoubted suicidal tendencies. As I felt it not safe to leave her in the hospital, I regret that I have found it necessary to place her under restraint.

Dr. Jordan, at my request, saw her on the 20th instant; we are both convinced that it will not be desirable for her to remain here, even should she recover.
