From ill health. Vide Jr Murray's Letter of the 24 November 1871, forming Enclosure in Despatch from the Secretary of State, No 144, of 29 November 1871..

Statement in terms of sec. viii of the Superannuation Act, of 1859, that the Applicant "has discharged his duties with diligence and fidelity, to the satisfaction of the Head Officer or Officers of his Department," to be signed by any two of such Head Officers, if there shall be more than one, or by such Head Officer, if there should be but one, together with "such a statement as will exhibit the merit, and value, and labour of the services of the Person recommended, embracing as long a period of his public service as can be authentically stated," together with observations as to Special Services, Suspension, Reprimand, &c., with full particulars of any injuries received on duty, or other claims or matters for consideration.

Dates of Commencement and Termination of the several Appointments held by Applicant, with their Emoluments, distinguishing Salary from other Allowances, and specifying such Allowances.

Colonial Surgeon from 11th December 1850. Vide Secretary of State to Governor No.73 of 23 Decem 1858) to 17 January 1872...

During the course of this service Dr Murray has drawn Salary as Colonial Surgeon, as follows:

(1.) from 20th February 1859 to 31st May 1861, at the rate of £800 per annum.

(2.) From 10th June 1861 to 31 December, 1866, at the rate of £500 per annum, with the privilege of Private Practice

(3.) From 1st January, 1867, to 17th January, 1872, at the rate of £700 per annum, with the like privilege of Private Practice...

Surgeon of the Lock Hospital from 15 May, 1859, to 14th December 1860; and again from 6th May 1861, to 17th May 1871, with a Salary at the rate of £180 per annum.

Whether holding any other Public Appointment, or receiving or claiming to receive any Public Money by Compensation, Half Pay or otherwise.

Has enjoyed Private Practice since June, 1861.
