I hereby Certify,
1st, That the above leave was granted to commence on the Twenty-Second Day of June, 1839 and end on the Twenty-first Day of June, 1860 on the ground of ill health and that the Medical Certificate produced is annexed, of which a Copy is annexed.

I hereby Certify that after a most careful examination of the Case of M. A. L'Ingles, I have come to the decided conclusion that he is now and must for some time past have been suffering from Bright's disease of the Kidney a disease in itself Exceedingly dangerous. After due consideration I have also concluded that it is absolutely necessary for him to proceed without loss of time to Europe, and recommend him to leave by next mail Steamer accordingly.
2d, That A. L. Inglis, Esquire, has served the Crown for 19 years, one month and 19 Days.
3d, That in the course of that Service he has been absent on leave on several occasions, at the dates and for the terms specified below, viz:
In the year 1858. Seven Days
4th, That provision has been made for the due execution of Inglis, Esquire's Office during his absence by the appointment of Mr. H. Newman, chief clerk in the Harbour Master's Department
NOTE.-Officers who may be under the necessity of applying to the Secretary of State for an extension of their leave of absence, are requested to annex this document to their applications; and in order to meet this contingency, it will be furnished to them in duplicate.
