Thereby certify that William Chapman has been under my care for some time past for derangement of the Liver and stomach accompanied with Rheumatism. He is at present suffering from his Liver and is obliged to take a move to the seaside. I consider it won't be injurious for him, in his present state of health, to travel on any journey or any long journey.
