I hereby Certify,
1st, That the above leave was granted to commence on the 5th Day of May 1859 and end on the [Day] of November 1860, on the ground of ill health, and that the Medical Certificate was produced of which a copy is annexed.
2d, That Mr Alfred Weatherhead has served the Crown for one year, ten months, and 4 Days.
3d, That in the course of that Service he has been absent on leave on several occasions, at the dates and for the terms specified below, viz:
In the year 1858. — 11 Days.
I certify that Dr Caldecott, late Acting Colonial Surgeon, has been attending Mr. A. Weatherhead, Clerk of the Supreme Court, for the last 9 months for Irritable Stricture of the Urethra and chronic disease of the Kidney's. The disease has latterly very much increased and on careful examination of Mr. Weatherhead, I am of opinion that he can derive benefit from further treatment in China and that an immediate return to England is absolutely necessary. I further declare my belief that he will not be able to resume his duties in less than one year.
