A FRENCH DOCTOR'S CRITICisu.
WRONG DIAGNOSIS.
One of the sections, of the British Medi- cal Association, which held its annual conference in Glasgow, was devoted on July 27th to tuberculosis.
Professor Erist, "of Paris, read a paper concerning the failure, in the earlier years of the war, to diagnose correctly lang. tuberculosis as distinct from other res- piratory diseases. This, he said, had forced upon the profession over the civil ized world the recognition of an urgent need for remodelling their diagnostic methods. In 1916 he was Army consultant cönduct- ing a tuberculosis cloaring station at Cou piigne. Out of 192 'men seat in with u diagnosis of lung tuberculosts from various Army units, ambulances, or hospitals, and kept under observation, the diagnosis was found to be justified in only fifty-three cases, which included arrested and healed, as well | as active, tuberculosis. One hundred and thirty-nine cases were undoubtedly non- tuberculous. During the summer and autumn of 1918 he acted in the same 'capacity at the Laennec Hospital for the troops stationed in Paris. "Of 382 mer. ex- amined, thirty-seven only, or 10.8 per cent... had lung tuberculosis; twenty-two wore not definitely diagnosed because the time they. were kept ander observation was too short; 283 tuberculous. “

Dr. Dickinson, tuberculosis officer of Newcastle-on-Tyne, gave him the following information concerning the er-Service men- who applied in Newcastle for sanatorium benefit between 1913 and 1919. Out of 287 meo, 167 had bacilli in their sputum and 120 lad nore. Of the latter, ninety- one were ultimately proved to be non- tuberculous. Among the 167 who had " bacilii fifty-three died, while among the one bandred and twenty whose sputim was free from bacilli only four died. The causes of For death in those four cases were bronchiectasis. influenza, sarcoma of the Jung, and „dis- seminated tuberculosis.

they had been the occasion of considerable 67, QUEEN'S ROAD, and perfectly useless expenditure of money at their own orat public expense; they lud been submitted to all the hardships, re- strictions, moral and material sacrifices which attended, the life of a tuberculous patient; and finally they had not even re- ceived the proper treatment which the real disease they suffered from required
"It would be absurd to contend,"he co- cluded, "that the diagnosis of tuberculosis, is justified only when bacilli have been demonstrated in the sputum. But the more ope studies this question of the discrimina tion of tuberculosis from other disease, and the more one has opportunities to follow up patients for long periods of time, the more ene seas how very frequent indeed, are the Cases, even the quite "early cases, of real tuberculosis where bacilli are found in the sputum when they are carefully looked for. I do not mean to say that a patient with positive signs of a localized lung lesion and, no demonstrable bacilli is certainly not tuberculous, I say that our duty in the absence of bacilli is to endeavour earnestly to ascertain whether some disease other than tuberculosis may not account for the symptoms and signs observed. Only if we do not succeed in our investigation are we entitled to admit. the diagnosis of tuberculosis."
