To be filed in and delivered by the employer to reach the Commissioner for Labour before overtime is worked on any day.

NOTE: Where authority is given under Regulation 19(4X9) of the Women and Young Persons (Industry) Regulations to work different parts of the factory, at different times of overtime in different periods, a separate form for the purpose of overtime, the part or section to which this Report relates should be clearly indicated.

註釋:「如工業處處長根據婦女及青年工人(工業)規例第十九(四)條批準某工廠於不同時期內在不同部門進行不同時段之超時工作,則本報告表須清楚註明與何部門或部份有關之超時工作。
