Prescribed for the purposes of regulation 19(4X8) of the Women and Young Person (Industry) Regulations.
To be filed in and delivered by the employer to reach the Commissioner for Labour before overtime is worked on any day.

NOTE: Where authority is given under Regulation 19(4X9) of the Women and Young Persons (Industry) Regulations to work different parts of the factory, at different times of overtime in different periods, a separate form for the purpose of overtime, the part or section to which this Report relates should be clearly indicated.
註釋:「如工業處處長根據婦女及青年工人(工業)規例第十九(四)條批準某工廠於不同時期內在不同部門進行不同時段之超時工作，則本報告表須清楚註明與何部門或部份有關之超時工作。
