(2) Subject to section 43 of the Ordinance, the register shall in respect of every existing discharge or deposit, contain a copy of Form 1, Form 2 or Form 8, as the case may be, as completed by the notifier and lodged with the Authority. (L.N. 203/88)
Registration of applications for licences
14. (1) The register shall contain the following particulars of every application to license a discharge or deposit from industrial premises or from a domestic sewage treatment plant which is made to the Authority under section 14 or 19 of the Ordinance-
(a) the date of the application;
(b) the name of the applicant;
(c) the location of the premises or plant;
(d) the name of the industrial enterprise or of the manager of the plant, if different from the applicant;
(e) the result of the application and the date on which the licence was granted or refused;
(f) the code number assigned by the Authority to the application.
(2) Subject to section 43 of the Ordinance, the register shall in respect of every application for a licence contain a copy of Form 5 or Form 10, as the case may be, as completed by the applicant and lodged with the Authority. (L.N. 203/88)
Registration of applications for variation or renewal
15. (1) The register shall contain the following particulars of every application for variation of an exemption relating to industrial premises or to a domestic sewage treatment plant which is made to the Authority under section 18 of the Ordinance-
(a) the date of the application;
(b) the name of the applicant;
(c) the location of the premises or plant;
(d) the name of the industrial enterprise or of the manager of the plant, if different from the applicant;
(e) the code number assigned by the Authority to the original notification;
(f) the code number assigned by the Authority to any previous variation of the exemption;
(g) the result of the application and the date on which the variation was allowed or refused;
(h) the code number assigned by the Authority to the application.
