WARNING-Section 48(1)(c) of the Travel Agents Ordinance makes it an offence punishable by a fine of $100,000 and to imprisonment for 2 years on conviction upon indictment or by a fine of $10,000 and to imprisonment for 6 months on summary conviction for a person who makes any false or misleading statement or furnishes any false or misleading information in connexion with any application for the issue of a licence.

個人或非立案法團機構申領牌照時須提供的詳細資料陳述書
*如你擬申請旅遊業監管局豁免你填寫此陳述書內某些資料的規定，請另紙寫明理由，並須註明有關條文編號、及加附你的簽署。
1. 你或你的管理人或職員有無經營旅遊業務的經驗?
2. 你是否會為他人安排旅程上的交通運輸工具，而旅程是以香港為起點，其後期在香港以外地方進行?
你是否會為他人訂租香港以外的住宿地方，而住宿費用由你或將由你支付?
你在香港經營業務過程中，是否曾與任何在香港以外地方領牌照的旅行社合作?
