STATEMENT IN SUPPORT OF AN APPLICATION BY
A COMPANY FOR RENEWAL OF A LICENCE
(To be completed in triplicate)

(a) Since the date of the company's application for a licence or last application for renewal (whichever is the later), has the company, or any of its directors, been convicted of any offence, other than a traffic offence, in Hong Kong or elsewhere? (answer "yes" or "no")
(b) If so, give the following details-
