MASSAGE ESTABLISHMENTS REGULATIONS

Citation
1. These regulations may be cited as the Massage Establishments Regulations.
Form of application for licence and renewal
2. The prescribed form of an application for a licence and for the renewal of a licence shall be that in Form 1 in Schedule 1.
Application for licence and renewal of licence
3. (1) An application for a licence and for the renewal of a licence shall be lodged with the licensing authority and shall be accompanied by 2 copies of a recent photograph of the applicant and such documents as may be prescribed in Part II of Form 1 in Schedule 1.
(2) Where the prescribed document is a sketch or plan, it shall be signed by the applicant certifying that it is a true copy.
Fee for licence
4. The prescribed fee for the issue of a licence and for the renewal of a licence shall be that in Schedule 2.
Licence
5. The prescribed form of a licence and a renewal of a licence shall be that in Form 2 in Schedule 1.
