4. (1) A certificate of posting under section 15(6) of the Ordinance shall be in accordance with Form 3 in the Schedule.
(2) A certificate of posting under section 17(2) of the Ordinance shall be in accordance with Form 4 in the Schedule.
(3) A certificate under section 21(1) of the Ordinance shall be in accordance with Form 5 in the Schedule.
5. An application for a certificate under section 22(4)(b) of the Ordinance shall be made to the Commissioner and shall be in accordance with Form 6 in the Schedule and the certificate shall be in accordance with Form 7 in the Schedule.
