Certified Copies of Accounts
There must be annexed to this return a written copy, certified both by a director and by the secretary of the company to be a true copy, of every balance sheet laid before the company in general meeting during the period to which this return relates (including every document required by law to be annexed to the balance sheet) and a copy (certified as aforesaid) of the report of the auditors on, and of the report of the directors accompanying, each such balance sheet. If any such balance sheet or document required by law to be annexed thereto or any such report is not in the English language there must also be annexed to that balance sheet a translation in English of the balance sheet, document or report certified in the prescribed manner to be a correct translation. If any such balance sheet as aforesaid or document required by law to be annexed thereto did not comply with the requirements of the law as in force at the date of the audit with respect to the form of balance sheets or documents aforesaid, as the case may be, there must be made such additions to and corrections in the copy as would have been required to be made in the balance sheet or document in order to make it comply with the said requirements, and the fact that the copy has been so amended must be stated thereon.

If any such register or part of any such register is kept outside Hong Kong, insert the address in Hong Kong where any duplicate thereof is kept.
Include also any mortgages and charges which would have been required to be so registered if created after 1st January 1912.
If the columns give insufficient space the particulars should be continued on a separate sheet. "Director" includes any person who occupies the position of a director by whatsoever name called and any person in accordance with whose directions or instructions the directors of the company are accustomed to act.
Full names, i.e. the present forename and surname, must be given, together with, in the case of a Chinese director, the Chinese characters therefor if such characters are contained in the register of directors—section 158 of the Companies Ordinance refers. "Forename" includes a Christian or given name and "surname" in the case of a person usually known by a title different from his surname means that title.
Any former forenames and surname must also be given, together with, in the case of a Chinese director, the Chinese characters therefor if such characters are contained in the register of directors—section 158 of the Companies Ordinance refers. Any alias must also be given.
"Former forename" and "former surname" do not include—
(a) in the case of a person usually known by a title different from his surname, the name by which he was known previous to the adoption of or succession to the title; or
