(3) e.g. value, percentage of alcohol content, whether denatured or not, etc. (4) Or, in the case of Removal Permit for Duty-paid Goods.To the DIRECTOR OF COMMERCE AND INDUSTRY.
I submit the following particulars for assessment of duty on the goods described below, which will be found at (1)

Note. This form must be submitted in duplicate, with any invoices, bills of lading, shipping orders, certificates of age or origin and other documents relating to the goods.
One copy only will be issued and, if it cannot be used, must be returned at once to the office of the Director.
