AFFIDAVIT IN SUPPORT OF APPLICATION FOR COMMITTAL OF DEBTOR FOR CONTEMPT UNDER SECTION 26 OR 55.

the Official Receiver of the estate of the said debtor [the trustee of the property of the said bankrupt] make oath, and say—
1. That the said debtor did attend at the first meeting of his creditors held on the day of 19 at, but did wilfully refuse to submit to be examined [or did not submit to examination] at such meeting in respect of his property [or his creditors], the submitting to examination being a duty imposed upon him by the Bankruptcy Ordinance (Chapter 6 of the Revised Edition).
