(a) Insert here "the time and place of a general meeting or adjourned general meeting" or as the case may be.
I, the trustee [or clerk to the trustee, as the case may be] in the above matter, make oath and say as follows-
1. That I did on the day of 19 send to each creditor who has proved in this matter, and also to all creditors mentioned in the debtor's statement of affairs, a notice of (a)
