must be declared as a Statutory Declaration before a Justice of the Peace or Notary Public and the certificate as to character and fitness shall be signed by a Justice of the Peace. Every application shall be accompanied by four passport size prints of a photograph of the applicant, one of which shall be affixed where indicated in the form and the applicant and the Justice of the Peace shall both sign their names on a portion of such prints.
11. Every application for admission to the register pur- suant to Section 8 of the Ordinance by persons claiming to be entitled to registration under Section 7 (a), (b) or (c) thereof shall be made to the Registrar in the form No. 5 in the appendix hereto which is obtainable from the Registrar; such application must be declared as a Statutory Declaration before a Justice of the Peace or a Notary Public and shall be accom- panied in the case of persons applying for registration under Section 7 (a) or (b) by four passport size prints of the applicant's photograph.
12. Registered dental surgeons whose names were on the Dental Register established under the provisions of the Dentistry Ordinance, 1914, on the coming into force of the Ordinance are exempted from the payment of the fee pre- scribed by regulation 7 and also of the annual fee prescribed by regulation 8 in respect of the year ending 31st December, 1940.
