The following words shall be inserted at the end of heading No. 1 :-
"and the office rates of premium for endowment assurances published and in force at the date of the valuation".
After the words "term of life" in line 1 of heading No. 2 there shall be inserted the words :-
"with muiform premiums throughout life or for whole terms of life with limited premiums ".
The following heading shall be inserted after head- ing No. 3:-
"4. The total amount assured under endowment assurances specifying sums assured, reversionary honuses, and office yearly premiums separately in respect of each Calendar year in which such assur- ances will mature for payment, and stating also the average age, as at the valuation date, in respect of each such Calendar year of maturity, distinguishing assurances with and without participation in profits."
The subsequent headings are numbered respectively 5, 6, 7, 8, 9, 10 and 11.
After the words "term of life" in line 2 of heading No. 5 as renumbered there shall be inserted the words :-
"and other than endowment assurances 11
Heading No. 6 as renumbered is amended by the substitution of the words "heading No. 5" for the words "heading No. 4" therein.
Heading No. 7 as renumbered is amended by the substitution of the words "heading No. 5" for the words "heading No. 4" therein.
Heading No. 9 as renumbered is amended by the substitution of the words "heading No. 8" for the words "heading No. 7".
Heading No. 10. Heading No. 10 as renumbered shall be deleted and the following shall be substituted in lieu thereof :-
Final note.
"The average rate of interest yielded by the assets whether invested or uninvested constituting the Life Insurance Fund of the Company, calculated up- on the mean fund of each year during the period since the last investigation."
The final note shall be deleted and the following substituted therefor :
Separate statements to be furnished for business transacted according to different office rates of pre- mium together with a statement of the manner in which policies on unhealthy lives are dealt with."
