ARTICLE 4
CONTENTS OF REQUESTS
In
all
cases requests
for assistance shall be
accompanied by :
(a) the name of the competent authority conducting the investigation, prosecution or proceedings to
which the request relates;
(b)
a
description
of
the
nature
of the
investigation, prosecution
or
proceedings,
including
summary a
of the relevant facts and
laws;
(c)
a
statement of the purpose for which the request
is made and the nature of the assistance sought;
(a)
a
of
the
need,
if any,
for
statement
confidentiality and the reasons therefor; and
