2. I can confirm that the above report should indeed be referred to as the 1992 report rather than the 1993 report. My mistake.
3. It seems that the report will be submitted to the UN Secretary-General in the next couple of months, although the exact date is uncertain. I shall certainly get the final version to you as soon as possible.
