Processing of Nomination Forms (GF 594)

Heads of Departments should check the accuracy of particulars of the officer entered in section A and complete section B of GF 594. The original copy of GF 594 should be sent to the Treasury as soon as possible under cover of a G.F. 121 (Despatch Schedule) which would show the names of all the officers concerned; the duplicate copy should be retained for departmental record and the triplicate copy should be returned to the officer concerned.

Processing of Revocation of Nomination Forms (GF 595)

Heads of Departments should complete section B of GF 595. The original copy of GF 595 should be sent to the Treasury as soon as possible under cover of a G.F.

121 (Despatch Schedule) which would show the names of all the officers concerned, the duplicate copy should be retained for departmental record and the triplicate copy should be returned to the officer concerned.

Subsequent Change of Marital Status

Henceforth, when

an officer reports a change of marital status (i.e. divorce or death of spouse) Heads

of Departments are requested to check whether he / she has nominated his / her spouse as recipient of death gratuity or exgratia payment.

In case there is a nomination, the officer concerned should be advised to revoke his/her nomination by completing and returning the Revocation of Nomination form (G.F. 595).
