Conditions 4 and 5: Improvement of Quality Management

This non-conformance was due to wrong interpretation of drawing by the contractor HCCM and the temporary arrangement for engineer to assume two independent job functions of directing and supervising the work. However, it was not discovered by checka before pouring of concrete. It showed that quality checks were not strictly enforced by the contractor, or adequately monitored by the JVC's QC surveillance team.
The underlying procedures and controls of both HCCM as well as the JVC-CM have since been examined by the contractor and the JVC management in depth. It has been concluded that, although the procedures are adequate in concept, it is necessary to implement a programme of improvement measures to ensure that they are used successfully and completely under the conditions on site. The major areas of improvement include:
Mobilization of additional qualified staff, and strengthening of quality supervision and control actions in both the HCCM ard the JE'S construction team and QC surveillance team.
Improvement of the existing procedures and controls (which are taken from French projects) to suit local conditions, including detailed instructions, more specific check procedures and improved records.
HCCM's permanent plan will be implemented progressively. In the meantime HCCM has implemented additional 100% checks and other interim measures to ensure that the works
works will be adequately controlled and monitored in line with the quality requirements.
In order to ensure that the work carried out by BCCM is subject to adequate surveillance and monitoring by the JVC, the JVC Construction Management has implemented measures including checks according to approved construction drawings.

ROOM and the JVC Management ha ve implementation of the interim measures in the last two weeks during which the reinforcement work in the second layer was carried out. The original and additional reinforcements in the second layer were 100% checked. It is concluded that with such interim measures the quality supervision and checks have been brought up to an acceptable level.
The JVC's QA Department conducted a further audit and is of the opinion that the interim measures to ensure good quality management work are effective. The JVC QC surveillance team will continue the measure of 100% checka until the HCCM's quality control system is completely installed and effective.
