(1) RECOMMENDATIONS

1.

In the light of my findings, I recommend to the Commissioner of Police that the following action to be taken:-

and

(a) that the procedure governing the arrest of police officers as adopted in this case be clarified and promulgated in orders on the lines described by me at paragraph (H)4 above;

(b) that orders be promulgated requiring the appointment

of a specified officer as the investigation officer at a very early stage in every police investigation, including enquiries into suicides. It should also be made clear that it is the responsibility of the appointed investigation officer to direct all action to be taken by subordinate officers and supporting staff;

(c) that no further consideration be given to the production of a 'check list' as an aid to investigation;

(a) that through the medium of training, stress be

laid on the need to ensure that in suicide cases even when the circumstances firmly point to suicide the 'scene of incident' investigation procedures are equivalent to those required for other homicide investigations, in order to completely eliminate other suggestions.

(e) that a direction be given to the effect that the

relevant Major Formation Commander shall at an early stage make a decision as to whether the investigation of a suicide case of unusual nature - such as this case is to be undertaken by Uniform Branch or C.I.D.
