(2) A visa application for resettlement in Japan shall be
submitted to a Japanese diplomatic or consular establish-
ment abroad which then refers the case to the Ministry of
Foreign Affairs in Tokyo.

(3) A local office of the United Nations High Commissioner for
Refugees is asked to take care of visa application procedures
on behalf of each applicant. This does not however proclude

the applicant himself from making the application.
(4) An application must be accompanied by the following documents
or evidence ( all in duplicate ) in addition to two sheets of
application forms and three passport-size photos.
(a) In case the applicant comes under the category
2-(1) of the preceding paragraph 2:
i. A written statement on the applicant's wish to resettle
in Japan (preferably to be accompanied by its English
translation).
ii. List of the applicant's family members (name, age,
family relationship, address of current residence
and profession).
iii. Personal history of the applicant.
iv. Copy of a document or evidence, if any, which may

serve to clarify the applicant's identity.
Record of medical examinations (inclusive of checks
on leprosy, mental disorder and epidemic disease).
